Individuals, families and communities face complex health challenges such as fractured health delivery systems and different health outcomes. This webinar will provide attendees with practical, evidence-based strategies to assess barriers, engage key stakeholders, and implement collaborative solutions in Age-Friendly Communities. Participants will gain an understanding of core concepts such as social determinants of health, interdisciplinary collaboration, policy considerations, and technology integration to improve health outcomes for older adults. The session will equip attendees with actionable tools to drive sustainable change in their communities.

Participants in this webinar will be able to:

  • Identify differences in healthcare access and outcomes related to aging, including barriers such as ageism, fragmented care models, and policy limitations.
  • Examine the intersection of aging and social determinants of health (e.g., housing, transportation, economic stability, and social support) to identify variations in health outcomes.
  • Evaluate evidence-based, upstream strategies for integrating community-based resources with healthcare systems to promote aging-in-place and improve long-term health outcomes.
